  • Coaches listen and assess to understand your unique needs and goals of care.
  • We use our subject matter expertise to assist you in the management of chronic health conditions, including congestive heart failure, chronic obstructive pulmonary, chronic kidney disease, diabetes, asthma, tobacco use and related conditions, heart disease, stroke, cancer, and others.
  • We educate you and your family/ caregivers to effectively self-manage your health issues including any social determinant of health needs. 
  • Coaches understand the science of health and the heart of the art of truly caring for you as an individual.
  • Coaches understand the health care system, so we can help navigate it with you. 

What to expect in a Health coaching session!

  1. 1-hour interactive telehealth session
  2. Plan of care expertly developed to pursue your vision of health
  3. -collaborative completion of a Health and Wellness Assessment Tool
    -education regarding your chronic health conditions and tools for self-management
    -goals of care discussions and advisement 
    -assistance with addressing your social determinant of health and community resource needs 
  4. -follow-up sessions at your convenience including weekend and evening hours to meet your lifestyle needs
