I am happy to be a health coach/life coach/personal consultant for care coordination or a designated Care Coordinator of services for anyone who is in the situation of: A parent is unavailable temporarily, and another family member is a temporary guardian who can collaborate with care providers and a mediation lawyer for designation of role and responsibilities, which can be documented and then brought to a family lawyer. Please know: a Primary Care Physician can offer a medical necessity report to help the relevant individual who might qualify for Medicaid benefits or other insurance benefits to help cover the cost of in home care services that are medical related supports. However, once you can let a clinical provider like a social worker at the locations know of your request to collaborate on care coordination, you can establish jurisdiction by explaining how your relate to the patient or client who requests services either You yourself request support or you are helping someone else. You can explain your jurisdiction through temporary guardianship or emergency guardianship if you are first, next of kin that is you are a relevant family member or friend or the family, and ask to be on the call list with name and phone number of you and or your designated counselor (www.psychologytoday.com) or children's primary care physician or explain who you want your provider to be. Finally you can sign a HIPAA release document to share information to support care of the patient or client amongst those persons.
