For some children that I am next of kin to, I can more proactively possibly be the care coordinator or even a temporary guardian, once a mediation lawyer meeting takes place.

Sometimes, births at home or on the way to a clinic happen.
In order to apply for a social security ID card you can get a medical note from primary care or other health providers and a school record either from preschool, homeschool for example, to help with the paperwork for obtaining the relevant ID cards.

Hi, how are you? These are some clinical and community based resources.
Please know: a Primary Care Physician can offer a medical necessity report to help the relevant individual who might qualify for Medicaid benefits or other insurance benefits to help cover the cost of in home care services that are medical related supports.

However, once you can let a clinical provider like a social worker at the locations know of your request to collaborate on care coordination, you can establish jurisdiction by explaining how your relate to the patient or client who requests services either You yourself request support or you are helping someone else.

You can explain your jurisdiction through temporary guardianship or emergency guardianship if you are first, next of kin that is you are a relevant family member or friend or the family, and ask to be on the call list with name and phone number of you and or your designated counselor (www.psychologytoday.com) or children's primary care physician or explain who you want your provider to be.

Finally you can sign a HIPAA release document to share information to support care of the patient or client amongst those persons.

When a person with a disability especially a child or adult with a disability requires a temporary guardian or guardian other than the parent, the client requesting support, that is the child or adult with a disability is referred to as the ward of the person with temporary guardianship or guardianship.

You can draft an example of what you are trying to do and have it notarized like at a postal office, or mailing service center and after talking with counselors and social workers and primary care physicians, and the client who can consent for themselves, even if they have a disability, or to the person representing them otherwise, if they have a disability and cannot consent for themselves, and then with a Family Lawyer present your request at a Family Court.
